LSM-11 issue

Hello,
just had a strange issue on a customer's car.
I bought and connected a new LSM-11 to a Motec M4 Pro with Advanced Tuning (Sensor select = 2 Wide Band).
During the mapping session the richer I could get DURING WOT tests was lambda 0.93 (while during gear-shifts - TPS=0% it was going down to 0.78...), but I later checked with an external LSU WB sensor that the real lambda was 0.60.
I then tuned the fuel, according to the LSU WB sensor, to lambda 0.80, but even in this case the LSM was showing 0.96...
Any idea?
Thanks

Does this ecu have the lambda option enabled? It is an extra on top of the advanced tuning upgrade...

Hi Adam,
yes, advanced and lambda option enabled...

I changed LSM-11, but same behaviour.
Then I tried to connect an external lambda reader to LA + and -, feeding a narrowband simulated output (sensor setup = 1 narrowband), but always got strange values (1.10 to 1.17).
Is it possible that M4 Lambda input is fried?

How far away is the sensor from the turbo outlet or exhaust port?

If it is too close then it will read lean

It is in the bottom part of the downpipe, 50cm from the turbine outlet...
moreover my own external lambda meter I used to map it, correctly reads around 0.98...
